CASE MANAGEMENT SERVICES
Case management is about responding to individual needs.
Neuro Case Management (NCM) Services provide the appropriate level of intervention necessary to enhance life style and to empower the individual and their family in accessing and utilising available community resources. Neuro Case Management Services aim to achieve maximum levels of functioning potential and increased social connectedness based on each individual’s needs.
All of NCM’s Case Managers are highly skilled health professionals and have significant experience in working with people as they move across the hospital, rehabilitation, residential and home environments.
Neuro Case Management offers a complete service whereby one Case Manager is the key contact who can assist with any or all of the following:
- Assessment of your service needs
- Identifying goals and development of care plans and rehabilitation programmes
- Monitoring and reviewing current or proposed packages of care
- Recruitment of all necessary staff – personal carers and support staff, rehabilitation assistants, Occupational Therapists, Physiotherapists, Psychologists, SLTs etc
- Provides information and assistance in identifying and accessing community support services as needed
- Advocating on your behalf
- Creation and maintenance of Risk Assessments
- Full family support services
- Arranging and coordinating services
- Assisting with health promotion and education
- Purchasing of care services on your behalf
- Counselling
- After hours telephone support service (24 hours per day)
- Support and development of compensatory strategies
- Case Management medico-legal reporting and expert witness services
- Enhancement of quality of life through skills training related to problem solving, seeking healthy recreational activities, and in development of meaningful relationships with family, friends and colleagues
Neuro Case management assistance can be for short periods or over the longer term for those clients with more complex and ever changing needs.
In consultation with you, your family and other professional teams, we can coordinate all of the required services and support your independence be it at home or in a residential setting.
